Our Family Caregiver Model enables members to receive care in the comfort of a family setting. This model allows family members to become the primary caregivers while receiving resources, training, and support from Advanced Care Support. This approach not only fosters familiarity but also strengthens family bonds.
Personal Care Alternative (PCA) Setting
For members who prefer more independence, our PCA service offers support in various settings, including their own apartment. Personal Care Assistants provide customized care to assist with daily living activities, enhancing self-sufficiency and promoting a higher quality of life.
Supported Community Connections (SCC)
Through our SCC program, members engage in enriching community activities and social interactions designed to build skills, confidence, and friendships. Whether it’s learning a new hobby, engaging in volunteer work, or developing life skills, our SCC service encourages meaningful connections within the community.
